To submit your evaluation, please enter your e-mail address in the box provided (to confirm you're actually taking the course you've selected), select the course from the drop down list, and respond to the questions by clicking on the appropriate button. If you want to change your selection, simply click on another button. Leave all the buttons blank if you don't want to respond to a particular question.
There are two sections to the evaluation: The Course, and The Professor. At the end of each, you have a scrolling box which allows you make detailed comments/suggestions.
When you are finished, you may reset the form or submit it. When you submit, the software will generate a report to the Associate Chair who collates the responses. As with the paper evaluations, your input will be confidential. Only the summary results and any individual comments (but not the identity of the author) will be forwarded by the Chair to the individual professor.
